Shoreline Orthopaedics accepts patients with a workers’ compensation claim.

The goal of treating workers’ compensation patients is to provide the highest quality of care to the patient and return them to the work force in a safe and timely manner. Shoreline Orthopaedics understands how important it is to each employer, nurse case manager, and/or claims adjuster to receive communication following a patient’s visit to our office. Patients will be provided with a Work Status Report at the end of each appointment and a copy will also be faxed to the employer or claims adjuster.

Our staff is dedicated to providing specialized service to meet the needs of both the patient and the employer, effectively and efficiently.

Most appointments require prior authorization.  We coordinate with the employer and the workers’ compensation insurance to obtain authorization in order to get patients scheduled without delay.

Once you have an authorized appointment, please download and print the Workers’ Compensation Information form and fill it out. Bring the completed form to your next appointment, or fax it to (616) 396-5720.
